My TDG essay introduces the concept but does not explain it in full. To answer your questions:

1. Most likely, the lowest tiers will be volunteers. The neighborhood representative position will likely take about 10–15 hours a month.
2. Each TDG is going to design itself. There will all sorts of experiments with this new way of democracy. We will learn how to make it work as we build it.
